DEADLINE INFORMATION:
Parents must have all immunizations up-to-date with medical documentation, or complete a Statement of Exemption signed and returned to the school nurse by OCTOBER 27, 2020* in order for your child to avoid being excluded from school.
*Note - Governor Wolf has extended the extending the deadline for Immunizations for two months after the start of school which would mean that by October 27th, all Twin Valley Students must have proof of immunizations submitted.
In accordance with the immunization requirements from the Pennsylvania Department of Health and State Board of Education all students grades K-12 must have the immunizations listed below to attend school.
A record of immunizations may be dropped off at your student's building main office. Contact your health provider or call 1-877-PA HEALTH for more information.
