DEADLINE INFORMATION:
In accordance with immunization requirements from the Pennsylvania Department of Health and State Board of Education all students grades (K-12) must have the following immunizations to attend school beginning in the 2019-2020 school year.
If your child is missing one or more of the immunizations listed below, they WILL NOT BE permitted to attend school until all immunizations are completed or a Statement of Exemption is completed.
Required Immunizations:
DTP/DTaP (4 doses) (1 dose on or after their 4th birthday)
Polio (4 doses) (4th dose on or after 4th birthday & at least 6 months after last dose)
MMR (2 doses) (measles, mumps, & rubella)
Hepatitis B (3 doses)
Varicella (2 doses) or evidence of chickenpox disease
Tdap (1 dose) required at 11-12 years of age to enter grades 7-12
Meningococcal (MCV) (2 doses) 1st dose at 11-15 years of age, required to enter grade 7, 2nd dose required at age 16 or entry into 12th grade.
